智能最佳kV扫描技术在胸部CT扫描中的应用价值

Application of CARE kV technique in chest CT scanning

摘要:

目的 探讨智能最佳kV扫描技术在胸部计算机体层摄影(CT)扫描中的应用价值。方法 140例怀疑胸部疾病患者随机分为实验组和对照组,分别采用智能最佳kV扫描技术和常规固定kV扫描技术进行胸部CT扫描,测量和计算肺尖部右侧胸大肌、气管分叉层面降主动脉和左房中部水平的平均像素值(CT 值)、图像噪声、信噪比和对比噪声比,采用5分制对两组图像质量进行评价,记录和比较两组容积CT剂量指数(CTDIvol)、剂量长度乘积(DLP)和有效剂量(ED)。结果 两组图像质量均能满足临床诊断需求。两组肺尖部右侧胸大肌、气管分叉层面降主动脉和左房中部水平的平均CT值比较差异均无统计学意义(P>0.05);实验组图像噪声均显著高于对照组(P<0.001),信噪比均显著低于对照组 (P<0.001);两组气管分叉层面降主动脉对比噪声比的差异无统计学意义(P>0.05)。实验组CTDIvol、DLP和ED均显著低于对照组(P<0.001)。结论 智能最佳kV扫描技术应用于胸部CT检查,能够在不影响诊断需求的同时显著减少受检者辐射剂量,具有潜在的临床应用价值。

Abstract:

Objective To investigate the application of CARE kV technique in chest computed tomography (CT) scanning. Methods A total of 140 patients suspected with chest diseases were randomly divided into experiment group and control group, and underwent chest scanning with CARE kV technique and routine kV technique respectively. The CT values, image noise, signal-to-noise ratios and contrast to noise ratios of apex of lung, level of descending aorta bifurcation of trachea and middle area of left atrium were measured. The image quality was assessed with 5-point scale. The CT dose indexes of volume (CTDIvol), dose length product (DLP) and effective doses (ED) were recorded and compared between groups. Results All the images in two groups could satisfy the requirement of diagnosis. There was no significant difference in the CT values of apex of lung, level of descending aorta bifurcation of trachea and middle area of left atrium between two groups (P>0.05). The image noise in experiment group was significantly higher than that in control group (P<0.001), and the signal-to-noise ratio in experiment group was significantly lower than that in control group (P<0.001). There was no significant difference in the contrast to noise ratio of level of descending aorta bifurcation of trachea between two groups (P>0.05). CTDIvol, DLP and ED in experiment group were significantly lower than those in control group (P<0.001). Conclusion CARE kV scanning in chest CT examination could significantly reduce the radiation dosage and satisfy the requirement of diagnosis, which has the potential of clinical value.
