U波倒置者行动态血压监测的意义

Significance of ambulatory blood pressure monitoring in patients with U wave inversion

摘要:

目的 分析在心电图提示U波倒置者中行动态血压监测是否有助于检出高血压。方法 选取心电图检查提示U波倒置的患者54例,行动态血压监测,分析U波倒置与24 h平均血压、日间平均血压、夜间平均血压、夜间平均收缩压、夜间平均舒张压、昼夜节律之间的关系。结果 24 h平均血压正常和升高者分别为16例(29.6%)和38例(70.4%),夜间平均血压正常和升高者分别为12例(22.2%)和42例(77.8%),差异均有统计学意义(P<0.01)。收缩压和舒张压昼夜节律正常与不正常者所占比例比较,差异均有统计学意义(P<0.01)。结论 U波倒置者行动态血压监测有助于发现未知的高血压患者及已知高血压患者中血压控制欠佳者,以指导临床更好地控制血压。

Abstract:

Objective To investigate whether ambulatory blood pressure monitoring in patients with inverted U waves before gynecologic operation is helpful to identify undiagnosed hypertension. Methods Fifty-four patients with U wave inversion received ambulatory blood pressure monitoring before gynecologic operation. The correlations among U wave inversion, 24-hour average blood pressure, diurnal blood pressure, nocturnal blood pressure, nocturnal systolic pressure, nocturnal diastolic pressure, and circadian rhythm were analyzed. Results For 24-hour average blood pressure, normal and elevated patients were 16 (29.6%) and 38 (70.4%)(P<0.01). For nocturnal blood pressure, normal and elevated patients were 12 (22.2%) and 42 (77.8%)(P<0.01). For systolic pressure, diastolic pressure, and circadian rhythm, normal and abnormal ratios were also significantly different (P<0.01). Conclusion Ambulatory blood pressure monitoring in patients with U wave inversion is helpful to identify patients with undiagnosed hypertension or uncontrolled hypertension and to achieve better blood pressure control.
