焦虑障碍患者无法忍受不确定性水平及相关因素

Level of intolerance of uncertainty and related factors in patients with anxiety disorders

摘要: 目的·探讨焦虑障碍患者无法忍受不确定性(intolerance of uncertainty,IU)水平及可能的影响因素。方法·收集符合《美国精神障碍诊断与统计手册(第4版)》(DSM- Ⅳ)诊断标准的广泛性焦虑障碍(generalized anxiety disorder,GAD)患者35例、惊恐障碍(panic disorder,PD)患者15例,同期募集正常对照组(HC组)对象31名。采用中文版无法忍受不确定性量表(Intolerance of Uncertainty Scale,IUS)评价被试的IU水平,采用汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)、汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)、躯体感知觉问卷(Body Perception Questionaire,BPQ)评估焦虑、抑郁症状和躯体感知觉特点。结果·GAD组和PD组IU水平均显著高于HC组(均P0.000),GAD组和PD组之间差异无统计学意义(P0.491)。GAD患者的IU水平与HAMA- 躯体焦虑(r0.377,P0.025)、BPQ- 觉察度(r0.342,P0.044)均呈正相关。结论·焦虑障碍患者的IU水平高于正常人群,GAD患者的IU水平可能与躯体焦虑及躯体感知-觉察度相关。

Abstract:

Objective · To investigate the level of intolerance of uncertainty (IU) and possible related factors in the patients with anxiety disorders. Methods · The patients’ diagnosis was made according to Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM- Ⅳ ). Thirty-five cases of generalized anxiety disorder (GAD), 15 cases of panic disorder (PD) and 31 healthy controls (HC group) were recruited at the same time. IU level was assessedusing the Chinese version of Intolerance of Uncertainty Scale (IUS). The characteristics of anxiety symptoms, depressive symptoms and body perception were assessedHamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD) and Body Perception Questionaire (BPQ), respectively. Results · The IU levels of both GAD group and PD group were significantly higher than those of HC group (P0.000). There was no significant difference in IU levels between GAD group and PD group (P0.491). The IU levels of GAD patients were positively correlated with HAMA-somatic anxiety (r0.377, P0.025) and BPQ-awareness (r0.342, P0.044). Conclusion · The patients with anxiety disorders have elevated IU levels, compared with healthy people. Possibly, somatic anxiety and body perception-awareness are related factors to IU in GAD patients.
