    Preliminary analysis of miRNA expression profile of gastric cancer tissues

    Objective To analyse the expression profile of specific microRNA (miRNA) of gastric cancer tissues. Methods miRNA microarrays were employed to detect the expression profile of miRNA of 21 paired gastric cancer tissues and corresponding paracancerous normal tissues, and preliminary bioinformatic analyses were conducted. The microarray results were validated by Real-Time PCR. Results It was revealed by miRNA microarray detection that there were 88 miRNA with differential expression in gastric cancer tissues and paracancerous normal tissues, among which miR-21, miR-196b, miR-301a, miR-431*, miR-550*, miR-18a and miR-135b were significantly up-regulated, and miR-139-3p, miR-628-3p, miR-596, miR-99b* and miR-638 were significantly down-regulated. The findings of RT-PCR on 4 miRNA (miR-181 and miR-19b of up-regulation and miR-134 and miR-31 of down-regulation) were in high concordance with microarray results. Conclusion There is specific miRNA expression profile of gastric cancer tissues, and these differentially expressed miRNA may serve as new biomarkers of gastric cancer.

    Objective To investigate the effect of CAG regimen of recombinant human granulocyte colony stimulating factor (rhG-CSF)+ cytarabine (Ara-C)+aclarubicin(ACR)on U937 cell line. Methods Leukemia cell line U937 was treated with rhG-CSF (50 ng/mL), Ara-C (10 nmol/L) and ACR (10 nmol/L) alone or jointly, and control group (without treatment by any of the drugs), Ara-C group, rhG-CSF group, ACR group, rhG-CSF+ACR group, Ara-C+ACR group, Ara-C+rhG-CSF group and Ara-C+rhG-CSF+ACR(CAG)group were established, respectively. Cell proliferation inhibition rates were calculated by cell count analyzer, and the expression of cell surface differentiation antigen CD11B, cell apoptosis rates and changes of cell cycle were determined by flow cytometry. Results In Ara-C group, Ara-C+ACR group, Ara-C+rhG-CSF group and CAG group, the percentages of cells in S phase were lower than that in control group 1 d after treatment. Two days after treatment, cell proliferation was significantly inhibited, the expression of CD11B significantly increased, and the expression of CD11B in CAG group was significantly higher than that in Ara-C group (P<0.05). Partial differentiation of cells was observed 6 d after treatment, with significant cell differentiation characteristics in CAG group. There was no significant difference in cell apoptosis rate among each group (P>0.05). Conclusion CAG regimen could induce differentiation of U937 cells in a time-dependent manner.

    Objective To investigate the effect of bone morphogenetic protein 2 (BMP2) small interfering RNA (siRNA) targeted suppression on osteogenic differentiation and calcification of human umbilical arterial smooth muscle cells (HUASMC) induced by interleukin-6 (IL-6). Methods Primary HUASMC cultured in vitro were served as study objective. RNA interference (RNAi) experiment platform was established, and transfection condition was optimized with FAMnegative siRNA. Primary HUASMC were transfected with four candidate siRNA of BMP2 gene, respectively, the expression of BMP2 mRNA was detected by Real-Time PCR, and active suppression siRNA was screened. The thawed primary HUASMC were divided into siRNA transfection group (transfection with active suppression siRNA) and mocking transfection group (control group, without transfection with siRNA), and both group were stimulated with recombinant human IL-6 (10 ng/mL). The expression of BMP2, bone specific alkaline phosphatase (BAP), osteocalcin (OC) and osteopontin (OPN) mRNA was detected by Real-Time PCR after simulation for 12 and 48 h. The thawed primary HUASMC were divided into siRNA transfection group, control group and blank group (without rhIL-6 stimulation, similar to control group for the other treatment). rhIL-6 (50 ng/mL) was added to siRNA group and control group for stimulation. The calcium deposition in cell layers was measured by o-cresolphthalein complexone method before stimulation, 2 d and 4 d after stimulation with rhIL-6, and the results were corrected by total protein content. Results The suitable RNAi experiment platform was successfully established. Twenty hours after stimulation with 10 ng/mL rhIL-6, the expression of BMP2 and BAP mRNA of HUASMC in siRNA transfection group was significantly lower than that in control group (P<0.05). Forty-eight hours after stimulation, the expression of OC and OPN mRNA in siRNA transfection group was significantly lower than that in control group (P<0.05). Before stimulation with 50 ng/mL rhIL-6, there was no significant difference in calcium deposition in HUASMC cell layers among siRNA transfection group, control group and blank group (P>0.05). For siRNA transfection group, calcium deposition in cell layers after stimulation with 50 ng/mL rhIL-6 for 2 d and 4 d was significantly lower than that in control group (P<0.05), and calcium deposition in cell layers after stimulation for 2 d was significantly higher than that in blank group (P<0.05). Conclusion Primary HUASMC cultured in vitro can lead to siRNA targeted suppression of BMP2 gene, and can significantly inhibit the osteogenic differentiation and calcification induced by IL-6.

    Objective To investigate the effect of mononuclear macrophage colony-stimulating factor (GMCSF) on the expression of vascular endothelial growth factor (VEGF) in fibroblasts, and explore the role of extracellular signal-regulated kinase (ERK) pathway in this process. Methods GMCSF and/or ERK pathway specific inhibitor (PD98059) were added to the culture fluid of human skin fibroblasts at exponential phase of growth, and blank control group, PD98059 group, GMCSF group and PD98059+GMCSF group were divided. Cells and supernatant were harvested from each group, the expression of VEGF mRNA and protein in human skin fibroblasts of each group were detected by RT-PCR and Western blotting, respectively, and the levels of VEGF protein in the supernatant were determined by ELISA. Results Compared with blank control group and PD98059 group, the expression of VEGF mRNA and protein in human skin fibroblasts and the levels of VEGF protein in the supernatant in GMCSF group were significantly increased (P<0.05), while the parameters in GMCSF+PD98059 group were significantly lower than those in GMCSF group (P<0.05). Conclusion GMCSF can up-regulate the expression of VEGF in human skin fibroblasts, and ERK pathway plays an important role in this process.

    Protective effect of lytic cocktail on lung tissues of severely scalded rats at initial stage

    Objective To investigate the protective effect of lytic cocktail on lung tissues of severely scalded rats at the initial stage, and explore the potential mechanism. Methods Sixty-six male SD rats were randomly divided into sham scald group (n=6, sham scald with subcutaneous injection of 12 mL/kg saline), scald group (n=30, third degree scald with subcutaneous injection of 12 mL/kg saline) and scald+lytic cocktail group (n=30, third degree scald with subcutaneous injection of 12 mL/kg lytic cocktail). Blood samples were obtained 3 h after scald simulation from sham scald group and 3 h, 6 h, 12 h, 24 h and 48 h after scald from scald group and scald+lytic cocktail group, and lung tissues were harvested after sacrifice from each group. Plasma levels of interleukin 1β (IL-1β), tumor necrosis factor α (TNF-α), interleukin 10 (IL-10), soluble intercellular adhesion molecule 1 (sICAM-1) and endothelin 1 (ET-1) were determined with ELISA, and the activity of myeloperoxidase (MPO) and the expression of CD68 in lung tissues were detected with enzymatic spectrophotometry and immunohistochemistry, respectively. Pathological changes in lung tissues were observed under light microscope with HE staining. Results The plasma levels of IL-1β, TNF-α, sICAM-1 and ET-1, and the activity of MPO and the expression of CD68 in lung tissues in scald+lytic cocktail group were higher than those in sham scald group and lower than those in scald group. The plasma level of IL-10 in scald group was higher than that in sham scald group and lower than that in scald+lytic cocktail group. There were significant differences in part of the above parameters among groups (P<0.05 or P<0.01). Compared with scald group 24 h after scald, pulmonary interstitial edema in scald+lytic cocktail group was alleviated at the corresponding time point. Conclusion Lytic cocktail can effectively alleviate the pulmonary interstitial edema and inflammation of lung tissues of severely scalded rats at the initial stage, and the potential mechanism may relate to the regulation of inflammation and reduction in activation and injury of endothelial cells.

    Objective To observe the effects of acute exhaustive running on rat cardiovascular ultrastructure and vessel function. Methods After adaptative running for 5 d, 24 male Wistar-Kyoto rats were assigned to sedentary control group (no more running, n=8), adaptative running group (adaptative running for another 5 d, n=8) and acute exhaustive running group (exhaustive running for another 5 d till refusal to running, n=8). After the end of running, the contraction and relaxation function of aortic vessels were assessed, and the ultrastructure of myocardium and aorta was examined under transmission electron microscope. Results Compared with sedentary control group and adaptative running group, the aortic contraction response to high potassium was not altered, while that to phenylephrine was significantly depressed in acute exhaustive running group (P<0.01). The endothelium-dependent vessel relaxation to acetylcholine was significantly enhanced in acute exhaustive running group (P<0.01), and was blocked by nitric oxide synthase inhibitor. Lysis of aortic endothelium, break of internal elastic lamina, infiltration of mononuclear macrophages around smooth muscles, degeneration in mitochondria and contraction band in myocardium in acute exhaustive running group were revealed by transmission electron microscopy. Conclusion Acute exhaustive running could lead to the impairment of vessel wall and myocardial mitochondria. The improvement in vessel relaxation and depression in contraction response may contribute to hypotension and sudden death after overexercise.

    Preparation and identification of 190CT3-myc fusion polypeptide

    Objective To construct a myc-tagged 190CT3 polypeptide (190CT3-myc) with genetic recombination method. Methods PCR was used to amplify cDNA sequences encoding 190CT3-myc fusion polypeptide, which were cloned to T vectors. After enzyme digestive identification of the sequences, 190CT3 eukaryotic expression vector was constructed and attached with myc fusion protein tag, and was transfected into Chinese hamster ovary (CHO) cells. After screening by G418, CHO cells which stably expressed target genes were obtained, and were identified by immunofluorescence method. 190CT3-myc fusion polypeptide was purified with antigen-antibody affinity chromatography column, and Coomassie brilliant blue staining and Western blotting were conducted after SDS-PAGE. Results cDNA fragment encoding 190CT3-myc was obtained by PCR amplification, and was confirmed by enzyme digestive identification of the sequences. Immunofluorescence method revealed that CHO cells transfected with recombinant plasmid stably expressed 190CT3-myc. Coomassie brilliant blue staining indicated the expression of fusion polypeptide 190CT3myc accounted for about 10% of the total protein in cells, with the purification ≥90%. Western blotting demonstrated that the relative molecular weight of purified fusion polypeptide was 17 000, which was in accordance with target protein. Conclusion 190CT3-myc recombinant polypeptide has been successfully prepared, which may provide new ideas for the drug target research of leukemia.

    Function change of pancreatic islet β-cells in first degree relatives with normal glucose tolerance in type 2 diabetes mellitus families

    Objective To investigate the function of pancreatic islet β-cells in first degree relatives with normal glucose tolerance in type 2 diabetes mellitus (T2DM) families. Methods The first degree relatives of T2DM families with normal glucose tolerance (T2DM first degree relatives group, n=94) and normal glucose tolerance subjects without diabetes mellitus family history (normal control group, n=98) were enrolled. Oral glucose tolerance test and serum insulin release test were taken for obtaining the whole body insulin sensitivity index [ISI(Matsuda)], early insulinogenic index and total insulinogenic index (IGI30 and IGI), respectively. Glucose disposition indexes (DI) including DI30 [ISI(Matsuda)×IGI30] and DI [ISI(Matsuda)×IGI] were calculated to evaluate the early and total insulin secretion of islet β-cells. Results ISI(Matsuda) in T2DM first degree relatives group and normal control group were 107.39±41.88 and 105.18±44.18, respectively, DI were 631.04±179.25 and 665.66±230.71, respectively, and there was no significant difference in ISI(Matsuda) and DI between these two groups (P>0.05). However, DI30 in first degree relatives group (1 248.44±894.41) was significantly lower than that in normal control group (1 558.35±1 015.66)(P<0.05). Conclusion There is early insulin secretion deficiency of pancreatic islet β-cells in first degree relatives with normal glucose tolerance in T2DM families, while the insulin sensitivity still remains normal.

    Systematic evaluation of selenium in treatment of autoimmune thyroiditis

    Objective To systematically evaluate the efficacy and safety of selenium in treatment of autoimmune thyroiditis (AITD). Methods Based on the principle and methods of Cochrane systematic review, the Cochrane Library, Medline, Embase, Chinese Biomedicine Database, China National Knowledge Infrastructure, VIP Database for Chinese Technical Periodicals and Wanfang Data were searched, and the randomized controlled trials (RTC) of selenium in treatment of AITD were included. Meta analysis was conducted for the data using RevMan 5.0 software. Results Seven RCT involved with selenium in treatment of AITD were included, and there were 513 patients in all. Meta analysis revealed that the positive rate of serum antithyroid peroxidase antibody (TPOAb) in selenium treatment group was significantly lower than that in control group treated with placebo [weighted mean difference (WMD)=-342.67, 95%CI: -433.33 to -242.02, P<0.000 01], and the healthy status of the former was better than the latter (RR=2.79, 95%CI: 1.21 to 6.47, P=0.02). The adverse effects were less and moderate after selenium treatment. Conclusion Meta analysis of this study indicates that selenium therapy for AITD is effective and safe. However, the evidence is not powerful enough due to the low quality of trials and limitation of sample number. The rigorously designed, multicenter, randomized and doubleblinded trials with large samples are needed for further research.

    Evaluation and MRI findings of occlusal splint in treatment of anterior disc displacement of temporomandibular joint

    Objective To investigate the outcomes of stabilization occlusal splint in treatment of anterior disc displacement of temporomandibular joint, and explore the role of MRI in evaluation of the therapeutic effect. Methods Thirty-three patients with anterior disc displacement of temporomandibular joint diagnosed by MRI and treated by stabilization occlusal splint were selected. According to the conditions of symptomatic relief, occlusal splints were worn for 6 months, from 24 h per day in the early stage to night hours only later. After treatment, mandibular movement function was assessed by oral opening distance, visual analogue scale (VAS) score was obtained, and a second MRI was performed for comparison with MRI findings before treatment. Results After treatment, the mean VAS score was 2.38±1.58, and was significantly lower than that before treatment (7.49±2.09)(P<0.001). The mean oral opening distance was (40.14±5.03) mm, and was significantly larger than that before treatment (26.5±5.12) mm (P<0.001). There was no significant change in MRI findings after treatment in most of the patients. Only two patients were observed to have reduced arthroedema by a second MRI, the articular discs recaptured to normal position in one of them, and the articular interlocking recovered to reducible articular disc displacement in the other. Conclusion Stabilization occlusion splint can significantly relieve the pain and improve the mandibular movement function in the treatment of anterior disc displacement of temporomandibular joint, and MRI findings can not be used as the single criterion for evaluation of therapeutic effect.

    Management of acute angle-closure glaucoma by phacoemulsification combined with goniosynechialysis

    Objective To investigate the clinical effects of phacoemulsification combined with goniosynechialysis in management of primary acute angle-closure glaucoma complicated with cataract. Methods The clinical data of patients with primary acute angle-closure glaucoma complicated with cataract were collected, and those (32 eyes) who received phacoemulsification combined with goniosynechialysis for uncontrolled intraocular pressure after general and topic medical management were selected. Operations were conducted within 72 h after acute episode for 29 eyes, and the other 3 eyes were operated 120 h after acute episode. The operation procedure was reviewed, and gonioscopic findings of the last follow up were recorded. Parameters of anterior segment structures of eyes including intraocular pressure, visual acuity, central anterior chamber depth, anterior chamber volume and anterior chamber angle width of examinations before operation and in last follow up after operation were analysed. Results Gonioscopy of last follow up revealed that iris root was flat, angle of anterior chamber was open, and there was much pigment on the surface of trabeculum. The angle of anterior chamber in 24 of the 32 eyes was totally open, with operations within 72 h after acute episode. Intraocular pressure was >50 mmHg (1 mmHg=0.133 kPa) in all the 32 eyes before operation, the mean intraocular pressure was (15.3±5.0) mmHg in the last follow up, the intraocular pressure was <21 mmHg in 28 eyes (all were performed operations within 72 h after acute episode), and the intraocular pressure was 21 to 27 mmHg in the 4 eyes. The last follow up after operation revealed that the best corrected visual acuity significantly elevated from that preoperation (the optimal corrected vision was <0.1 in those with operations 120 h after acute episode). Parameters of anterior segment structures of eyes after operation were significantly better than those before operation (P<0.001). Conclusion Phacoemulsification combined with goniosynechialysis is effective in management of primary acute angle-closure glaucoma complicated with cataract, while the opportunity and indication of surgery should be properly determined.

    Analysis of residual dizzy symptoms after successful particle repositioning maneuver in patients with benign paroxysmal positional vertigo

    Objective To observe the characteristics and possible relevant factors of residual dizzy symptoms in patients with benign paroxysmal positional vertigo (BPPV) after successful particle repositioning maneuver. Methods The clinical characteristics of 486 patients with BPPV were retrospectively analysed. Patients were followed up for 3 months after successful particle repositioning maneuver, the characteristics and duration of BPPV of residual dizzy symptoms were observed, and the relevant factors were analysed. Results Among the 486 patients, 167(34.4%) were males and 319 (65.6%) were females, with the mean age of (56.9±12.9) years and mean disease duration of (9.1±8.4) d. Two hundred and eight-one patients (57.8%) had residual dizzy symptoms after successful particle repositioning maneuver, and the symptoms lasted for (27.27±23.32) d (2 to 90 d). The factors of female gender, longer disease duration and number of repositioning therapy ≥3 were significantly associated with residual dizzy symptoms. Conclusion Prompt diagnosis and effective particle repositioning maneuver can reduce the incidence of residual dizzy symptoms in patients with BPPV.

    Clinical significance of NT-proBNP in early evaluation of left heart function for patients with hypertension combined with diabetes mellitus

    Objective To investigate the clinical significance of N-terminal pro-B-type natriuretic peptide (NT-proBNP) in early evaluation of left heart function for patients with hypertension combined with diabetes mellitus. Methods One hundred and twenty-three patients with hypertension and without clinical evidence of heart failure were divided into hypertension combined with diabetes mellitus group (n=63) and simple hypertension group (n=60). Plasma NTproBNP concentrations were measured, and echocardiography was performed in all patients. The correlation between plasma NT-proBNP concentration and parameters of echocardiography was explored by multiple regression analysis. Results The plasma NT-proBNP concentration in hypertension combined with diabetes mellitus group was (376±50) ng/L, and was significantly higher than that in simple hypertension group (98±16) ng/L (P<0.01). Echocardiography revealed that the cardiac structure and cardiac function were normal in simple hypertension group, while the left atrial diameter (LAD), left ventricular end-diastolic diameter (LVED), left ventricular posterior wall thickness (LVPWT), interventricular septal thickness (IVST) and heart rate (HR) significantly increased, and the left ventricle ejection fraction (LVEF) significantly decreased in hypertension combined with diabetes mellitus group (P<0.05 or P<0.01). Multiple regression analysis indicated that NT-proBNP was negatively related to LVEF (R2=-0.30, P<0.05), but positively related to LAD, LVED, HR, IVST and LVPWT (R2=0.18, 0.11, 0.09, 0.20 and 0.13, P<0.05 or P<0.01)in hypertension combined with diabetes mellitus group. Conclusion Plasma NT-proBNP may serve as a parameter of early evaluation for left heart function in patients with hypertension combined with diabetes mellitus.

    Objective To analyse the relationship between estimated glomerular filtration rate (eGFR) and 99mTc-glomerular filtration rate (99mTc-GFR), and explore the suitable methods for eGFR of Chinese inpatients with chronic kidney disease (CKD). Methods One thousand and thirty-three inpatients with CKD were selected, 99mTc-GFR was measured by 99mTc-diethylenetriaminepentaacetic acid (99mTc-DTPA) dynamic renal imaging, and serum creatinine (SCr) was determined by enzymatic method. Cockcroft-Gault formula, modification of diet in renal disease (MDRD) equation, abbreviated MDRD equation, MDRD equation for Chinese, abbreviated MDRD equation for Chinese and CKD-EPI equation were employed for calculation and body surface area (BSA) was used for correction of eGFR (C-G GFR, 7GFR, aGFR, c-7GFR4, c-aGFR4 and CKD-EPI-GFR, respectively). The relationship between 99mTc-GFR and eGFR was analysed. Results Correlation regression analysis revealed that the correlation coefficients between C-G GFR, 7GFR, aGFR, c-7GFR4, c-aGFR4, CKD-EPI-GFR and 99mTc-GFR were 0.805, 0.797, 0.783, 0.791, 0.777 and 0.800, respectively, and the differences were -2.46, -14.24, -16.53, -20.92, -22.92 and -13.56, respectively. There was approximate hyperbolic relation between 99mTc-GFR and SCr. Conclusion C-G GFR is most closely related to 99mTc-GFR among eGFR from SCr, C-G formula may be a suitable method for eGFR of Chinese inpatients with CKD when SCr is measured by enzymatic method.

    Objective To observe the distribution of CD4+Foxp3+ regulatory T cells (CD4+Foxp3+ Treg) and the subsets in CD4+T cells in peripheral blood of adults with minimal change disease (MCD), and explore the significance of CD4+Foxp3+Treg in the pathogenesis of MCD. Methods Twenty-seven adults with MCD initially diagnosed by renal biopsy were selected. The antibody combination of Foxp3-FITC/CD45RA-PE/CD3-ECD/CD4-PC7 was adopted and flow cytometry was employed to determine the percentage of CD4+Foxp3+ Treg and the subsets in CD4+T cells in peripheral mononuclear cells (PBMCs). The expression of Foxp3, RORc, TGF-β1, IL-6, IL-17A and IL-23 mRNA in PBMCs was detected by Real-Time PCR. Besides, normal controls were established. Results Flow cytometry revealed that CD4+Foxp3+Treg were divided into three subsets: CD45RA+Foxp3lowcells(Fr.Ⅰ, resting Treg), CD45RA-Foxp3highcells (Fr.Ⅱ, active Treg) and CD45RA-Foxp3lowcells (Fr.Ⅲ). The percentages of cells of Fr.Ⅰ+Fr.Ⅱ and cells of Fr.Ⅱ in CD4+ T cells in MCD group were significantly lower than those in control group (P<0.01). The expression of Foxp3 mRNA in PBMCs in MCD group was significantly lower than that in control group (P<0.05), while the expression of RORc mRNA in PBMCs in MCD group was significantly higher than that in control group (P<0.05), and the expression of IL-6, IL-17A and IL-23 mRNA was significantly higher than that in control group (P<0.05, P<0.01). Correlation analysis revealed that the expression of IL-17A mRNA was negatively related to the percentage of cells of Fr.II in CD4+T cells in MCD group (r=-0.81,P<0.05). Conclusion In adults with MCD, there is decreased expression of Treg and active Treg subsets and increased expression of Th17-related gene and cytokines in peripheral blood, which suggests that the down-regulated expression of Treg Foxp3 mRNA and Treg/Th17 imbalance might be involved in the pathogenesis of MCD.

    Analysis of risk factors and prognosis in patients with posterior circulation ischemic stroke

    Objective To investigate the distribution of risk factors and its influence on prognosis in patients with posterior circulation ischemic stroke (PCS). Methods The data of demographics, NIHSS scores on admission and vascular risk factors were collected from 129 patients with PCS confirmed by clinical and imaging examinations. Patients were followed up for 3 months for prognosis information. The prevalences of risk factors were calculated, and their relationship with prognosis was analysed.ResultsAmong the investigated risk factors, hypertension (83.7%), hyperlipoidemia (56.6%), cigarette smoking (39.5%) and hyperglycemia (39.5%) were the most prevalent ones. Patients with 0 to 7 risk factors accounted for 4.65%, 10.85%, 23.26%, 27.13%, 24.81%, 4.65%, 3.88% and 0.78% of the total, respectively, and most patients had hypertension and another one or more risk factors. NIHSS scores on admission, instead of vascular risk factors, were independently correlated with prognosis (OR=1.58, 95% CI, 1.28-1.96). Conclusion Risk factors in patients with anterior circulation ischemic stroke or PCS are similar, and should be controlled in patients with PCS. Risk factors have impact on the incidence of PCS, while do not determine the prognosis of patients.

    Objective To evaluate the effects of different dental filling materials in wedge-shaped defect restorations of maxilla premolars (54|45)on periodontal tissues.MethodsForty patients (160 teeth) with wedge-shaped defect (54|45)of maxilla premolars were selected, and four different maxilla premolars of each patient were randomly filled with glassionomer (GI group), rensin-modified glass-ionomer (RMGI group), polyacid-modified composite resin/compomer (PMCR group) and composite resin (CR group), respectively. The periodontal health indexes including amount of gingival crevicular fluid and activity of aspartate aminotransferase and alkaline phosphatase in gingival crevicular fluid of tested teeth were recorded before restorations and one week, one month and six months after restorations. Results Compared with those before restorations, the amount of gingival crevicular fluid significantly increased in each group after restorations, and the activity of aspartate aminotransferase and alkaline phosphatase in gingival crevicular fluid also significantly increased (P<0.05). Before restorations and one week after restorations, there was no significant difference in each index among four groups (P>0.05). One month and six months after restorations, the indexes of GI group were significantly higher than those of the other groups (P<0.05), while the indexes of PMCR group were significantly lower than those of the other groups (P<0.05). Conclusion All the four commonly used dental filling materials have adverse effects on periodontal tissues after restorations of wedge-shaped defect (54|45), and adverse effects from restorations with GI may be most significant.

    Objective To explore the strategy of microvascular decompression (MVD) on patients with hemifacial spasm complicated with trigeminal neuralgia. Methods Nine patients with hemifacial spasm complicated with trigeminal neuralgia treated with MVD were selected. The conditions of cerebrovascular diseases were examined by three dimensional time of flight (3D-TOF) nuclear magnetic resonance (MRI). The intraoperative findings as well as outcomes were analysed. Results Preoperative MRI revealed that the big twisted vertebral arteries (VA) deviated to the affected side, and even the contralateral VA moved to the affected side. It was observed during operation that VA directly compressed both facial and trigeminal nerves in 3 patients, VA lifted the posterior inferior cerebellar arteries (PICA) or the anterior inferior cerebellar arteries (AICA) in 5 patients which compressed both facial and trigeminal nerves, and PICA and superior cerebellar artery compressed facial and trigeminal nerves respectively in one patient. The symptoms of hemifacial spasm and trigeminal neuralgia disappeared immediately after operation in 8 patients. For the other patient, the pain relieved one week after operation, and the symptoms completely disappeared 3 months after operation. There was no reoccurrence during the follow-up. Conclusion Large and dislocated VA may often be the direct and indirect responsible vessels for hemifacial spasm complicated with trigeminal neuralgia. The key to the operation is to move the VA proximally instead of stucking barely the pad between the nerve and vessel.

    Objective To improve the diagnosis rate of fine needle aspiration biopsy in thyroid malignant tumors through comparing the features between cytopathology and histopathology, together with the characters of ultrasound images. Methods The data of 28 cases of thyroid cancer diagnosed by fine needle aspiration biopsy were collected, and the cytological and histological features were reviewed. The ultrasound images of the cases were also analyzed. Results Twenty-six cases of papillary carcinoma and 2 cases of medullary carcinoma diagnosed by fine needle aspiration biopsy were confirmed by histological diagnosis afterward. In all the node samples of papillary carcinoma, characteristic nuclear grooves and pseudo-inclusion bodies were found in nuclei. Of the 28 cases of thyroid cancer, the diameters of nodes were ≥2 cm in 13 cases and <2 cm in 15 cases. Fifteen cases had a single node while 13 cases had multiple nodes. Twenty-two cases (78.57%)had calcification in nodes, 16 (72.73%)of which were microminiature. Twenty-three cases (82.14%)showed irregular edges that were histologically confirmed as the performance of the infiltrative growth of the tumor and the reaction of the mesenchyme. Ultrasound image analysis revealed that most nodes had gravel-like calcification, with irregular and spiculated edges. Conclusion Fine needle aspiration biopsy under ultrasound guiding could improve the accuracy in finding micro nodes. The analysis of characters of edge and calcification in thyroid nodes on ultrasound images could increase the diagnosis rate of fine needle aspiration biopsy.

    Objective To investigate the relationship between endometriosis and/or adenomyosis-related infertility and facial melanin pigment, and explore the phenotypic characteristics and possible mechanism of endometriosis and/or adenomyosis. Methods One hundred and fifty infertile women with endometriosis and/or adenomyosis (endometriosis and/or adenomyosis group) and two hundred infertile women without endometriosis and/or adenomyosis (control group) were selected. The age, body mass index (BMI), serum basic estradiol (E2) level and incidence of facial melanin pigment (positive rates) of two groups were analysed, and the serum E2 level, cancer antigen 125 (CA125) level and positive rates of anti-endometrial antibody (AEmAb) were compared between patients with positive facial melanin pigment and those with negative facial melanin pigment in endometriosis and/or adenomyosis group. Results The positive rate of facial melanin pigment of endometriosis and/or adenomyosis group was significantly higher than that in control group (47.33% vs 19.00%)(P<0.05). The positive rates of facial melanin pigment of patients of different age stages in endometriosis and/or adenomyosis group were significantly higher than those in control group (P<0.05). There was no significant difference in age, BMI and serum basic E2 level between two groups (P>0.05). The serum basic E2 level, CA125 level and positive rate of AEmAb of endometriosis and/or adenomyosis group were higher than those in control group, with no significant difference between two groups (P>0.05). Conclusion Endometriosis and/or adenomyosis may be related to facial melanin pigment. Whether facial melanin pigment is independent of basic estrogen level and reflects the condition of endometriosis and/or adenomyosis needs to be further studied.

    Objective To observe the influence of postoperative application of parecoxib sodium combined with patient-controlled epidural analgesia (PCEA) on hepato-renal function and drug consumption of PCEA. Methods Sixty women undergoing epidural anesthesia were randomly divided into 2 groups (n=30). Patients in parecoxib sodium+PCEA group were intravenously injected with 40 mg parecoxib sodium after abdominal closure, and were then managed with PCEA pump (0.005% morphine+0.24% ropivacaine mesylate), while patients in PCEA group were only managed with PCEA pump (0.005% morphine+0.24% ropivacaine mesylate) after abdominal closure. Parameters of hepato-renal function were measured 24 h after operation. Visual analogue scale (VAS) scores were obtained 4, 8, 12 and 24 h after operation, and palping times of PCEA and drug consumption of PCEA were recorded meanwhile. Results There was no significant change in parameters of hepato-renal function after operation in both groups(P>0.05). VAS scores, palping times of PCEA and drug consumption of PCEA 4, 8, 12 and 24 h after operation in parecoxib sodium+PCEA group were significantly lower than those in PCEA group(P<0.05 or P<0.01). There was no additional analgesic medication for both groups. Conclusion Parecoxib sodium for postoperative analgesia has no significant influence on hepato-renal function, while it can obviously enhance the analgesic effect and decrease the drug consumption of PCEA.

    Objective To observe the systemic acute inflammatory reaction after full-mouth subgingival scaling and root planning (FM-SRP) or quadrant subgingival scaling and root planning (Q-SRP) in patients with moderate or severe periodontitis. Methods Twenty patients with moderate or severe periodontitis were randomly divided into FM-SRP group and Q-SRP group (n=10). The baseline clinical parameters of plaque index (PI), gingival index (GI), bleeding on probing (BOP), probing depth (PD) and clinical attachment loss (CAL) were measured and compared. Serum C-reactive protein (CRP) was detected before and 2 h after scaling. Questionnaire survey was conducted to record the gingival pain during and after operation, the use of analgesic drugs and the conditions of acute inflammatory reaction such as fever. Results There was no significant difference in the baseline clinical parameters between the two groups (P>0.05). There was no significant difference in CRP levels between FM-SRP group and Q-SRP group before scaling [(3.17±0.54) mg/L vs (3.42±0.71) mg/L] and 2 h after scaling [ (3.17±0.54) mg/L vs (3.46±0.83) mg/L](P>0.05), and there was no significant change after scaling within groups (P>0.05). Questionnaire survey revealed that there was no significant difference in operative and postoperative reactions between groups, and there was no significant changes after scaling within groups (P>0.05). Conclusion Both FM-SRP and Q-SRP may not lead to systemic acute inflammatory reaction in patients with moderate or severe periodontitis, and proper subgingival scaling modality may be chosen according to the clinical practice.

    Objective To detect the daily dietary intakes of trace elements (copper, iron and zinc) in adult residents of mining area by duplicated diet and weighting dietary record, and explore the correlation between these two methods. Methods A total of 569 adult residents in 180 families were randomly selected from Dahongshan Copper Mine in Yunnan Province, and dietary investigations were conducted for three consecutive days. Daily dietary intakes of copper, iron and zinc were determined by weighting dietary record in all residents, and 60 residents randomly selected from these 569 residents were also investigated by duplicated diet. Data from “Chinese Dietary Reference Intakes (DRIs)” were served as controls. Results It was revealed by both methods that dietary copper, iron and zinc intakes in females were significantly lower than those in males (P<0.05), while daily intakes in both males and females were lower than the tolerable upper intake levels. Among the residents investigated by both methods, the median intakes of copper, iron and zinc detected by weighting dietary record were 1.53 mg/d, 21.76 mg/d and 11.54 mg/d, respectively, and were significantly higher than those detected by duplicated diet (1.27, 16.29 and 7.56 mg/d, respectively)(P<0.05). The data obtained by two methods were positively correlated (r=0.537, 0.656 and 0.560, respectively, P<0.05). Conclusion Daily dietary intakes of copper, iron and zinc in adult residents of Dahongshan Copper Mine are lower than the tolerable upper intake levels. The data determined by weighting dietary record are higher than those by duplicated diet, and the findings obtained by these two methods are significantly correlated.

    Analysis of efficiency of causality judgement indicators of adverse drug reactions

    Objective To analyse the efficiency of causality judgement indicators of adverse drug reactions (ADR). Methods Causality judgement indicators of ADR were collected by literature review, 14 indicators (reaction time, withdrawl symptom, a second drug application, similar report, another evidence, similar response, dose change, plasma drug concentration, placebo, combined medication, family history, past history, baseline disease and non-drug factor) were determined as key indicators for efficiency of causality judgement of ADR by expert interview, and questionnaires were designed. Sixty-five staff from Shanghai Food and Drug Administration and medical units were surveyed, and 61 (94%) effective questionnaires were recovered. Factor analysis was conducted on the basis survey findings, and the role of each indicator in causality judgement was obtained by variance calculation.The minimum number of causality judgement indicators of ADR was determined by dimension reduction method. Results Kaiser-Meyer-Olkin and Bartlett tests revealed that there existed relevance among various indicators. When the number of indicators reduced to 7, the efficiency was ≥85%. The relationship among 7 indicators (past related indicator, information evidence indicator, withdrawl reaction indicator, plasma drug concentration indicator, time indicator, control indicator and similar case indicator) were illustrated by score function. Judged by 7 indicators, nearly 15% information was not included, and might lead to false judgement. Conclusion More factors should be included in the causality judgement of ADR in the establishment of compensation system for ADR to ensure social justice.

    Objective To discuss the methods of diagnosis and treatment of malignant melanoma of penis. Methods The clinicopathological characteristics, procedures of diagnosis and treatment, and prognosis of 4 cases of malignant melanoma of penis were retrospectively analysed. Results All the 4 cases were confirmed as malignant melanoma of penis by pathological examinations. One case was classified as stageⅠ (T1N0M0) and the other three cases stage Ⅲ (T3N2M0, T3N2M0 and T4N3M0, respectively). Partial penectomy was performed in three cases and radical penectomy in the other one, and all were conducted bilateral inguinal lymphadenectomy. Patients were followed up for 6 to 60 months, and two died. Conclusion Preexisting nevi may be risk factors for development of malignant melanoma of penis, and early diagnosis and proper surgical therapy are of paramount importance in improving the prognosis.

    Objective To improve the method for primary cultivation of rat pulmonary microvascular endothelial cells (PMVECs), and identify the cultivated cells. Methods Method for primary cultivation of rat PMVECs was improved from the procedures of animal selection, sampling, perfusion and tissue piece pasting. After subculturing, freezing and thawing, PMVECs were observed by inverted microscopy for morphology and growth behavior, identified by immunohistochemical staining for expression of factor Ⅷ and CD31, and observed by fluorescence microscopy for binding with lectin from BSI (FITC-BSI binding assay). Results Under inverted microscope, the cultivated cells exhibited as polygon or fusiform, and displayed typical cobblestone-like morphology after fusion to monolayer, with contact inhibition. Immunohistochemical staining revealed that the expression of factor Ⅷ and CD31 was positive. Besides, there were positive findings for FITC-BSI assay. Conclusion The improved method for primary cultivation is easy to handle, with favourable repeatability and success rate, and the morphological and biological characteristics of cultivated cells are in line with PMVECs.

    Objective To establish an improved culture method for obtaining oligodendrocyte precursor cells (OPCs) with high purity. Methods Mixed glial cells of cerebral cortex of newborn (1 to 2 d after born) Sprague-Dawley (SD) rats were primarily cultured for 8 d, and cells were cultured and purified by orbital shaking, differential adhesion and trypsin digestion in combination with defined culture media. Cell morphology was observed by light microscope with trypan blue staining. Two days after culture, cells were identified by immunofluorescence staining with A2B5 and DAPI, and purity analysis was conducted. Results It was observed by light microscopy that the cytomembrane of cultured cells was intact, with no cell injury. Purity analysis after immunofluorescence staining revealed that the ratio of number of A2B5-positive cells to that of DAPI-positive cells was 98.14%. Conclusion OPCs with high purity can be obtained by the improved methods of mixed glial cell culture and purification.

    Prune belly syndrome (PBS) is a congenital developmental anomaly of rare occurrence, which is characterized by the clinical manifestations of absence of abdominal musculature, urinary system abnormity and bilateral cryptorchidism. A case of PBS is reported, and some issues about PBS are discussed on basis of literature review in this paper.
