血清miR-499与急性冠脉综合征患者冠状动脉病变严重程度的相关性分析

摘要

目的·探讨急性冠脉综合征(acute coronary syndrome,ACS)患者血清中miR-499表达水平对冠状动脉病变严重程度的预测价值。方法·纳入就诊于上海交通大学医学院附属新华医院的ACS患者74例,根据Gensini评分将患者分为轻度组(30例)、中度组(21例)及重度组(23例)。荧光实时定量聚合酶链反应(real-time quantitative polymerase chain reaction,qPCR)技术检测血清miR-499的表达量,同时检测肌钙蛋白(cTnI)、肌酸激酶同工酶(CK-MB)、肌红蛋白(Mb)、脑钠肽(BNP)、左心室射血分数(LVEF)等实验室指标。采用Spearman相关分析评估miR-499表达水平与Gensini 评分、冠状动脉病变血管支数的相关性。受试者操作特征(receiver operating characteristic,ROC)曲线分析miR-499对冠状动脉病变严重程度的评估价值。结果·miR-499的表达水平随着冠状动脉病变程度的加重呈上升趋势,组间差异有统计学意义(P=0.000)。miR-499表达水平与Gensini 评分、cTnI水平呈正相关(r值分别为0.331、0.143,P<0.05),与LVEF无相关性(P>0.05);Gensini评分与cTnI、LVEF均无相关性(P>0.05)。ROC曲线分析结果显示,miR-499评估冠状动脉病变严重程度的AUC为0.802(95%CI 0.698~0.906),灵敏度为65.2%,特异度为80.4%。miR-499表达水平随着冠状动脉病变支数的增多呈升高趋势,且与冠状动脉病变支数呈正相关(r=0.368,P<0.05)。结论·ACS患者血清中miR-499表达水平与冠状动脉病变严重程度及病变支数相关,对冠状动脉病变严重程度具有一定评估价值。

Abstract

Objective ·To explore the value of miR-499 expression levels in the serum of patients with acute coronary syndrome (ACS) in predicting the severity of coronary artery disease. Methods ·Seventy-four patients with ACS admitted to Xinhua Hospital, Shanghai Jiao Tong University School of Medicine were included and divided into a mild group of 30 cases, a moderate group of 21 cases, and a severe group of 23 cases according to the Gensini score. Real-time quantitative polymerase chain reaction (qPCR) technology was used to detect the expression level of serum miR-499, while troponin (cTnI), creatine kinase isoenzyme (CK-MB), myoglobin (Mb), brain natriuretic peptide (BNP), left ventricular ejection fraction (LVEF), and other laboratory indicators were measured. Spearman correlation analysis was used to analyze the correlation between the expression level of miR-499 and Gensini score, as well as the number of vessel branches in coronary artery disease. The evaluation value of miR-499 in assessing the severity of coronary artery disease was analyzed by using receiver operating characteristic (ROC) curve. Results ·The expression level of miR-499 increased with the severity of coronary artery disease (P=0.000). The expression level of miR-499 was positively correlated with Gensini score and cTnI (r=0.331, r=0.143, P<0.05), respectively, but not with LVEF (P>0.05), while Gensini score was not correlated with cTnI and LVEF (P>0.05). ROC analysis showed that the AUC of miR-499 for assessing the severity of coronary artery disease was 0.802 (95% CI 0.698‒0.906), with sensitivity of 65.2% and specificity of 80.4%. The expression level of miR-499 increased with the increase of the number of coronary artery lesions, and was positively correlated with the number of coronary artery lesions (r=0.368, P<0.05). Conclusion ·The expression level of miR-499 in the serum of ACS patients is correlated with the severity and number of coronary artery lesions, which has a certain evaluation value in evaluating the severity of coronary artery lesions.

急性冠脉综合征(acute coronary syndrome,ACS)是严重的心血管疾病,给家庭和社会带来了极大的经济负担。ACS患者的临床表现变异较大,若不能及时诊断和治疗,可发生严重的心肌梗死、心力衰竭或猝死1。近年来,ACS的发病率和病死率呈显著上升趋势,并日益年轻化2。因此,尽早评估ACS患者冠状动脉病变的严重程度及分层,并积极予以治疗,可极大程度地改善预后。目前,临床上常用的确诊冠状动脉病变严重程度的金标准为冠状动脉造影3;但其费用昂贵,操作有创,且注射造影剂可能引起过敏性休克、肾脏损伤、穿刺部位出血或感染等4。因此,很难快速确定手术的必要性和价值。目前,急需寻找其他具有辅助诊断价值的血液生化指标,以便在不需要冠状动脉造影的情况下可以快速判断冠状动脉病变的严重程度。多项研究5-7表明,某些miRNA在心肌细胞中特异性表达。例如,miR-499与心肌损伤相关,并且参与心肌祖细胞向心肌细胞的分化8。研究5表明,在冠状动脉缺血1 h后,心肌富含的miR-499在外周循环中表达。有研究9-10已证实,miR-15、miR-378循环表达水平与冠状动脉狭窄的严重程度有关。然而,目前对ACS患者血清中miR-499的表达量与冠状动脉病变严重程度相关性的探索较少。本研究检测了ACS患者血清中miR-499的表达水平,并根据Gensini评分和病变血管支数对其进行分组,分析血清中miR-499水平与ACS患者冠状动脉狭窄程度之间的相关性,以期为尽早评估冠状动脉病变严重程度提供依据,进而指导临床早干预、早治疗。

1 对象与方法

1.1 研究对象

纳入2018年7月—2019年10月上海交通大学医学院附属新华医院收治的ACS患者74例(包括急性心肌梗死61例、不稳定心绞痛13例)。根据Gensini评分将患者分为轻度组(Gensini评分<60)30例、中度组(Gensini评分60~90)21例、重度组(Gensini评分>90)23例。纳入标准:①符合ACS诊断标准11。②年龄>18岁。③已接受冠状动脉造影检查。排除标准:冠状动脉痉挛或微血管病变引起缺血性胸痛;合并重肝肾功能障碍、全身免疫性疾病、恶性肿瘤、瓣膜性心脏病、血液系统疾病等。

1.2 临床资料收集

收集患者的临床资料,包括性别、年龄、冠心病史、高血压史、糖尿病史、吸烟史、冠状动脉造影结果等;收集入院即刻血常规、肌钙蛋白(cardiac troponin,cTnI)、肌酸激酶同工酶(creatine kinase-MB,CK-MB)、肌红蛋白(myoglobin,Mb)、脑钠肽(brain natriuretic peptide,BNP)、左心室射血分数(left ventricular ejection fraction,LVEF)等实验室指标。

1.3 样本收集与储存

采集患者血样2~3 mL至非抗凝管中,4 ℃下离心(1 509.3×g)10 min,制备血清;转至无RNA酶 EP管中,-80 ℃冰箱保存,待测。按照RNA 提取试剂盒操作步骤提取血清中总RNA,利用反转录试剂盒进行小RNA浓缩。反转录试剂盒和荧光实时定量聚合酶链反应(real-time quantitative polymerase chain reaction,qPCR)试剂盒均购自日本TaKaRa公司。以U6作为内参对照,应用ABI7500实时荧光定量PCR仪(Applied Biosystems,美国)进行分析,获得扩增产物CT值,采用2-ΔΔCT法分析miRNA的相对表达量。PCR扩增miRNA反应条件:变性95 ℃,10 s;qPCR×40个循环,95 ℃ 5 s,60 ℃ 20 s;溶解曲线,95 ℃ 60 s,55 ℃ 30 s,95 ℃ 30 s。实验重复3次,取平均值。miR-499、U6引物由上海生工生物工程有限公司合成。U6引物序列(5'→3')为CG CTTCACGAATTTGCGTGTCAT;miR-499引物序列(5'→3')为AGCGTTAAGACTTGCAGTGATGTT。

1.4 Gensini评分

冠状动脉病变严重程度采用Gensini评分表示。①定量评定:对狭窄程度最严重处进行评分,直径<25%计1分,25%≤直径<50%计2分,50≤直径<75%计4分,75≤直径<90%计8分,90≤直径<99%计16分,≥99%计32分。②病变血管系数:左主干为5;左前降支近段为2.5,中段为1.5,远段为1;第一对角支为1,第二对角支为0.5;左回旋支近段为2.5,远段和后段降支均为1,后侧支为0.5;右冠近、中、远段和后降支均为1。Gensini评分=定量评分×病变血管系数。

1.5 统计学分析

应用GraphPad Prism 7.0和SPSS 23.0软件处理数据。定性资料用例数及百分比(n,%)表示;定性资料以x±s的表示,采用t检验进行组间比较;采用Spearman相关分析评估miR-499表达量与Gensini评分及冠状动脉病变支数的相关性。采用受试者操作特征(receiver operating characteristic,ROC)曲线下面积评估血清miR-499表达量对ACS患者冠状动脉病变严重程度的诊断价值。P≤0.05表示差异有统计学意义。

2 结果

2.1 miR-499在不同冠状动脉病变严重程度患者中的表达差异

轻、中、重度组患者的Gensini评分分别为35.57±18.54、77.26±8.52、119.1±26.12,差异有统计学意义(P<0.05)。3组患者一般资料及部分实验室指标比较,差异无统计学意义(P>0.05)。miR-499在轻、中、重度3组患者血清中的表达水平,随着冠状动脉病变程度加重呈升高趋势,组间差异有统计学意义(P=0.000)。详见表1

2.2 miR-499Gensini评分与各指标的相关性

Spearman分析结果显示,miR-499与Gensini评分呈正相关(r=0.331,P=0.000)。cTnI的临床实测值不符合正态分布,对数据进行Ln转换后进行分析。结果显示,cTnI与miR-499表达水平呈正相关(r=0.143,P=0.001);LVEF与miR-499表达水平无相关性(r=0.005,P=0.554);Gensini评分与cTnI和LVEF均无相关性(r=0.030,P=0.114;r=0.017,P=0.264)。详见图1

2.3 miR-499表达水平对ACS患者冠状动脉病变严重程度的评估价值

ROC曲线分析结果显示,miR-499表达水平评估ACS患者冠状动脉病变严重程度的AUC为0.802(95%CI 0.698~0.906)(P=0.000)。当约登指数取最大值时,截断值为5.83,灵敏度为65.2%,特异度为80.4%(图2)。

2.4 不同冠状动脉病变支数患者血清miR-499表达水平

将不同冠状动脉病变支数患者血清中miR-499表达量进行比较,结果显示:单支病变患者miR-499表达量(3.65±1.85)低于双支病变患者(5.53±2.44)和多支病变患者(5.83±2.37),差异有统计学意义(均P=0.020);但双支病变患者与多支病变患者的血清miR-499水平比较,差异无统计学意义(P>0.05)。miR-499的表达量与冠状动脉病变支数呈正相关(r=0.368,P<0.05)。

3 讨论

miRNA参与ACS发生和发展的多个阶段,包括脂代谢紊乱、内皮受损、粥样硬化斑块的形成及其破裂出血等12-14。WANG等15通过结扎大鼠的冠状动脉左前降支,发现在相应的缺血心肌区域miR-499的表达水平升高,非缺血区域的miR-499表达量较低。 miR-499可能是从受损心肌细胞中释放后进入血液循环。可能的机制是miR-499通过抑制心肌细胞的程序性死亡因子4(PDCD4)和磷酸丝氨酸酸性簇排列蛋白2(PACS2)表达来保护心肌,从而抑制心肌细胞凋亡。研究16结果显示,与非心源性胸痛患者相比,miR-106b/25簇、miR-17/92a簇、miR-21/590-5p家族、miR-126和miR-451在ACS患者血浆中的表达水平显著升高。在急性心肌梗死(acute myocardial infarction,AMI)患者血清中,miR-1、miR-208、miR-133a、miR-499的表达水平显著升高17-18。DEVAUX等19发现AMI患者胸痛后3 h内miR-499水平显著升高,并与高敏肌钙蛋白(hs-TnT)呈正相关;miR-499诊断AMI的阳性率达到93%,其灵敏度和特异度高于hs-TnT然而,这些研究大多集中在ACS的急性期。一个理想的生物标志物不仅应该具有较高的敏感度和特异度,而且应该可以对心肌损伤的程度具有预测价值。本研究旨在探讨心肌特异性miR-499是否与冠状动脉病变的严重程度有关。

Gensini评分系统是冠心病研究领域使用最广泛的量化方法,不仅用于量化狭窄程度,而且可以准确、客观地反映冠状动脉不同部位的狭窄严重程度20。本研究根据Gensini评分将ACS患者分为3组,比较各组患者血清中miR-499的表达量。结果提示miR-499的表达量随着Gensini评分的升高而上升,在3组患者中的表达差异有统计学意义;而cTnI在3组患者中的表达无明显差异。Spearman分析结果显示,血清miR-499与Gensini评分、cTnI均呈正相关;Gensini评分和miR-499均与LVEF无明显相关性。可见,miR-499与心肌损伤的程度和病变血管的严重程度具有一定的相关性。

ROC曲线分析结果显示,miR-499对ACS患者冠状动脉病变严重程度具有评估价值。进一步的分析中,按照病变的血管支数将患者分为单支病变、双支病变和多支病变组。结果显示,双支病变和多支病变患者miR-499的表达水平明显高于单支病变患者,但双支病变与多支病变患者间的miR-499水平并无显著差异。相关性分析显示,miR-499表达水平与病变血管支数呈正相关。以上结果提示,miR-499具有用于预测血管狭窄严重程度的潜力。

本研究探索了血清miR-499与ACS病变严重程度的相关性,结果提示miR-499在一定程度上可预测ACS患者的血管狭窄程度,可以为复杂的经皮冠状动脉介入治疗提供参考。但本研究入选的均为ACS患者,并未区分不稳定型心绞痛、急性非ST段抬高型心肌梗死以及急性ST段抬高型心肌梗死;而临床上,对于不同类型的ACS患者所采取的治疗策略并不相同,故miR-499在其中扮演的角色也不同。如何建立miR-499与冠心病治疗策略的流程模型,仍需要进一步研究。
