单孔胸腔镜治疗肺隔离症15例报道

Report of 15 cases of single-port video-assisted thoracoscopic surgery for pulmonary sequestration

摘要: 目的·探讨单孔胸腔镜手术在肺隔离症治疗中的可行性。方法·回顾性分析上海交通大学医学院附属仁济医院2014年1月—2018年12月收治的15例肺隔离症(叶内型)病例的临床资料。所有病例均接受单孔胸腔镜肺叶切除;其中左肺下叶切除13例,右肺下叶切除2例。结果·15例患者中有胸膜粘连11例;以单孔胸腔镜成功切除肺叶12例,因全胸膜腔粘连中转多孔胸腔镜切除肺叶3例,无中转开胸手术病例;无围术期并发症,无围术期死亡。15例患者的平均手术时间(96.7±27.8) min,术中平均出血量(75.3±45.5) mL,术后胸腔引流管平均置管时间(5.2±2.0) d,术后平均住院天数(6.2±2.2) d。结论·单孔胸腔镜治疗肺隔离症是安全、有效、可行的手术方式。

Abstract:

Objective · To investigate the feasibility of single-port video-assisted thoracoscopic surgery (VATS) for pulmonary sequestration. Methods · Fifteen cases with intralobar pulmonary sequestration in Renji Hospital, Shanghai Jiao Tong University School of Medicine January 2014 to December 2018 were reviewed. All the cases had underwent lobectomysingle-port VATS, including 13 cases of left lower lobectomy and 2 cases of right lower lobectomy. Results · Pleural adhesion was found in 11 of the 15 cases. Lobectomy was successfully performedsingle-port VATS in 12 cases. Three cases were transferred to multi-port VATS becaof severe pleural adhesion. There was no case conversed to thoracotomy in the study. There was no mortality and complication during the perioperative period. For the 15 cases, the operation time was (96.7±27.8) min, the bleeding volume was (75.3±45.5) mL, the postoperative intubation was (5.2±2.0) d and the postoperative hospital stay was (6.2±2.2) d. Conclusion · Single-port VATS treatment for pulmonary sequestration is safe, effective and feasible.
