冠状动脉旁路移植术后中青年患者社会参与水平及影响因素的调查研究

Study on social participation and influencing factors in young and middle-aged patients after coronary artery bypass grafting

摘要: 目的·调查冠状动脉旁路移植术(coronary artery bypass grafting,CABG)后中青年患者出院后3个月的社会参与水平,并分析其影响因素。方法·2018年10月—2019年7月于上海交通大学医学院附属瑞金医院采用便利抽样选取行CABG的18~65岁患者259名,采用自主参与问卷调查患者出院后3个月的社会参与水平;同时采用一般情况问卷、全面健康素养测量量表、社会支持评定量表、冠心病自我管理量表、医学应对问卷获取患者的社会人口学信息、疾病相关信息、健康素养、社会支持、自我管理能力、疾病应对方式。结果·CABG术后中青年患者出院后3个月的社会参与得分为(46.58±8.83)分。多元逐步回归分析结果显示,疼痛(β6.191)、生活自理能力(β-0.270)、医疗资源可及性(β-3.228)、支持利用度(β-1.992)、社会支持(β-0.439)和批判性健康素养(β-1.325)是社会参与的影响因素。结论·在CABG术后中青年患者出院后的3个月内,应通过改善患者的疼痛情况、提高照护者的支持作用及社区可利用医疗资源、提供获取健康信息的可靠途径等方法促进患者的社会参与水平。

Abstract:

Objective · To investigate the social participation level of young and middle-aged patients with coronary artery bypass grafting (CABG) 3 months after discharge and analyze the influencing factors. Methods · Oct. 2018 to Jul. 2019, 259 patients aged 18-65 after CABG were selected in Ruijin Hospital, Shanghai Jiao Tong University School of Medicineconvenient sampling. Impact on Participation and Autonomy Questionnaire (IPA) was used to investigate the social participation level of the patients 3 months after discharge. General Situation Questionnaire, All Aspects of Health Literacy Scale, Social Support Rate Scale, Coronary Artery Disease Self-management Scale and Medical CopingModes Questionnaire were used to measure the patients social demographic information, disease-related information, health literacy level, social support, self-management level and disease coping style. Results · The score of social participation of young and middle-aged patients with CABG 3 months after discharge was 46.58±8.83. The results of stepwise regression showed that pain (β6.191), life self-care ability (β-0.270), medical resource accessibility (β-3.228), support utilization (β-1.992), social support (β-0.439), and critical health literacy (β-1.325) were the influencing factors of social participation. Conclusion · Within 3 months after the discharge of young and middle-aged CABG patients, the social participation level of the patients should be promotedimproving the pain situation of the patients, improving the support role of the caregivers, making of the medical resources in the community, and providing a reliable way to obtain health information.
