点阵CO2激光治疗女性压力性尿失禁的研究进展

摘要：

压力性尿失禁（stress urinary incontinence，SUI）常见于中老年女性，随着人口老龄化的增加，其发病率也不断攀升。临床上，轻中度SUI的主要一线治疗为盆底肌训练，但需医患双方投入大量的时间和精力。近年来，伴随着激光治疗SUI研究的日益增多，经阴道点阵CO₂激光疗法的可行性得到了初步证实。该疗法可快速促进被CO₂激光照射的热损伤区的弹力纤维和胶原蛋白的增生，增加尿道支撑结构的厚度及弹性，从而增强尿道的稳定性，以改善机体的控尿功能。目前，针对该方面的临床研究多为小样本、非随机化试验，尚无法得出对其疗效和安全性的有效结论。该文就点阵CO₂激光治疗女性SUI的具体机制及临床研究进展进行综述，以期为该疗法的应用提供理论基础与临床证据参考。

Abstract:

Stress urinary incontinence (SUI) is common in middle-aged and elderly women, and its incidence is also rising with the growth of aging population. Clinically, the main first-line treatment for mild to moderate SUI is pelvic floor muscle training, but it requires a lot of time and attention from both doctors and patients. In recent years, more and more studies have been conducted to explore the laser treatment of SUI, and the feasibility of transvaginal fractional CO₂laser treatment for SUI has been preliminarily confirmed. Fractional CO₂ laser can rapidly promote the proliferation of elastic fiber and collagen in the fractional photothermal area, and increase the thickness and elasticity of urethra supporting structure, thus increasing the stability of urethra and improving control of urination. However, most clinical studies in this field are non-randomized trials with small samples, and it is not yet possible to draw effective conclusions on its efficacy and safety. This paper reviews the specific mechanism and clinical research progress of fractional CO₂ laser in the treatment of female SUI, and provides theoretical basis and clinical evidence reference for the application of this therapy.
