结构化饮食管理课程对2型糖尿病血糖控制及饮食行为的影响

Effects of structured nutrition education on glycemic control and diet behaviors in patients with type 2 diabetes mellitus

摘要:

目的 观察结构化饮食管理课程在2型糖尿病治疗中的效果。方法 236例2型糖尿病患者随机分为常规饮食课程组(UNT组)和结构化饮食管理课程组(SNT组),UNT组仅接受常规饮食指导,SNT组在常规饮食课程的基础上给予结构性饮食教育指导。分别在基线及课程干预后6个月时,对两组空腹血糖、餐后血糖、糖化血红蛋白等临床生化指标以及知识掌握度和健康饮食行为指标进行统计学分析。结果 与基线数据比较,SNT组干预后6个月时的空腹血糖、餐后血糖和糖化血红蛋白以及知识掌握度和饮食行为均有明显改善(P<0.05)。与UNT组比较,SNT组在糖化血红蛋白、知识掌握度及健康饮食行为执行等方面的提升更为显著(P<0.05)。结论 结构化饮食管理课程对糖尿病的控制及行为的改善有明显促进作用。

Abstract:

Objective To investigate the effects of structured nutrition education in the treatment of patients with type 2 diabetes mellitus. Methods Two hundred and thirty-six patients with type 2 diabetes mellitus were randomly divided into structured nutrition team (SNT group) and usual nutrition team (UNT group). Patients in UNT group only received usual nutrition education, while those in SNT group received structured nutrition education in addition to usual nutrition education. The clinical biochemical parameters including fasting plasma glucose, postprandial plasma glucose and glycosylated hemoglobin, knowledge of nutrition and parameters of diet behaviors in two groups were analysed at baseline and 6 months after intervention. Results Compared with the baseline, the fasting plasma glucose, postprandial plasma glucose, glycosylated hemoglobin, knowledge of nutrition and diet behaviors significantly improved 6 months after intervention in SNT group (P<0.05). Compared with UNT group, SNT group achieved significantly better results in glycosylated hemoglobin, knowledge of nutrition and diet behaviors (P<0.05). Conclusion Structured nutrition education may play a role in the improvement of glycemic control and diet behaviors.
