实时三维超声心动图评估儿童川崎病左室收缩不同步的应用价值

Application value of real-time three-dimensional echocardiography in assessment of left ventricular asynchrony in children with Kawasaki disease

摘要:

目的 探讨实时三维超声心动图对儿童川崎病(KD)左室收缩不同步性评估的应用价值。方法 收集KD患者60例,健康志愿者40名。分析所有患者的二维经胸超声心动图Simpson法所测射血分数值并与三维超声心动图整体射血分数值比较。实时三维超声心动图分析患者左室17节段时间-容积曲线图,并与正常对照组对比。结果 KD患者二维Simpson法所测射血分数值与三维测量值呈中等程度相关。KD患者17节段时间-容积曲线呈方向不同的波形,其排列紊乱、不规则,到达最小容积的时间较离散。结论 17节段时间-容积曲线可作为评价左室心肌收缩不同步性的有效指标。

Abstract:

Objective To explore application value of real-time three-dimensional echocardiography in assessment of left ventricular asynchrony in children with Kawasaki disease (KD). Methods Sixty children with KD were analyzed retrospectively, and 40 healthy volunteers were enrolled as control group. The two-dimensional echocardiographic features and ejection fractions measured by Simpson's method of 60 cases were analyzed and compared with the global ejection fractions measured by three-dimensional echocardiography. The regional volume-time curves of 17 segments were acquired automatically by real-time three-dimensional echocardiography and compared to the control group. Results In 60 cases, the ejection fractions measured by Simpson's method correlated moderately with the global ejection fractions measured by real-time three-dimensional echocardiography; and the regional volume-time curves of 17 segments arranged in disorder of different directions; and the time to the point with minimal systolic volume was discrete. Conclusion The regional volume-time curves of 17 segments are important in evaluating the left ventricular systolic synchrony.
